Kelsey Seybold Clinic is Houston's premier multispecialty group practice, founded in 1949 by Dr. Mavis Kelsey in Houston's famous Texas Medical Center. This position works under the supervision of the Medical Director for Hospital and Supportive Medicine in the planning, implementation and evaluation of medical and nursing care for patients with complex care needs. The Care Manager - Nurse Practitioner assesses the physical condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories. The Care Manager orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. The Care Manager may prescribe and/or recommend medical drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy and with attention to efficacy, safety, and cost. The Care Manager formulates and implements comprehensive outcomes-based patient care plans based on patient assessments, scientific rationale, standards of care, and professional practice guidelines. The Care Manger initiates timely consultation and referral when the problem exceeds the scope of practice and/or expertise.
Conducts in home and in facility visits to assesses the physical condition of patients by performing comprehensive or problem-focused physical examinations and obtaining medical histories. Orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. Prescribes and/or recommends drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy and with attention to efficacy, safety, and cost Assists in closing quality care gaps (screenings and labs.) Identifies diagnoses and refers patients for complex care management and active chronic condition management.
Identifies patients with urgent and emergent situations and works with the Care Navigation team to ensure the appropriate and timely intervention by the member’s PCP and/or other members of Care Management Team. Identifies referral needs and educates members and families on topics related to health conditions and community resources as applicable.
Participates in weekly case conference with the Medical Director of Hospital Services and Supportive Medicine to review cases and develop care plans for complex patients. Makes rounds at facilities as needed or requested by Medical Director.
Masters’ Degree in Nursing – NP or APRN; 3 years’ related work experience; Licensed to practice as NP in state of Texas; DEA license; Current prescriptive authority; BCLS; ACLS.
Effective verbal and written communication skills and the ability to present information clearly and professionally.
Must be able to work with hours that sometimes vary, make home visits and travel in the greater Houston area daily. Some evenings and weekend work will be required
